2021 Holiday Basket Online Application

All applications must be received on or before Friday, October 22, 2021 no later than 4 p.m.

Please note that any and all information is subject to verification. Recipients will be on a first come - first-served basis. You can also complete the paper application HERE if you wish to do so.

If your application is verified and approved, you will receive a YELLOW POSTCARD in the mail the first week of November. You must present this YELLOW POSTCARD to receive your food gift card.
